Individual: The individual is the person requiring care or support. An advocate may act on behalf of an individual.

• Carry out the plan of care or support within the limits of own role
• Lead others to carry out their responsibilities when implementing a plan of care or support
• Adjust the plan in response to changing needs or circumstances

Learning Outcome 5 

• Agree on methods for monitoring the way a care plan is delivered
• Collate and record monitoring information from agreed sources and agree on any changes
• Seek agreement with the individual and others about who should be involved in the review process and the criteria to judge the effectiveness of the care plan
• Seek feedback from the individual and others about whether the objectives of the plan have been met
• Work with the individual and others to agree on any revisions to the plan

• Reducing the level of support to reflect increased independence
