  • Adjust the plan in response to changing needs or circumstances

  • Agree on methods for monitoring the way a care plan is delivered
  • Collate and record monitoring information from agreed sources and agree on any changes
  • Seek agreement with the individual and others about who should be involved in the review process and the criteria to judge the effectiveness of the care plan
  • Seek feedback from the individual and others about whether the objectives of the plan have been met
  • Work with the individual and others to agree on any revisions to the plan
